Hours of Operation

Monday - Friday
1:00 pm - 5:00 pm

Medical patients are seen Tuesday and Thursday evenings from 6:00 pm – 8:30 pm and the fourth Wednesday from 3:00 pm – 5:30 pm.  We have a pediatrician available once a month.

Dental patients are seen Mondays, Tuesdays, Wednesdays and Thursdays, at designated times throughout the day.

To Schedule an Appointment

Anyone without medical or dental insurance can receive care at the center; however, priority is given to those with the most financial difficulty and most urgent medical need.

To schedule a medical appointment, you can call 317-272-0708.  We require patient to bring proof of income.  If you do not have income, please be prepared to bring a copy of your year end taxes.

To schedule a dental appointment, you must come into the office and fill out paperwork prior to being scheduled for an appointment.  We require patient to bring proof of income.  If you do not have income, please be prepared to bring a copy of your year end taxes.
